| Brief Description of Services Provided: | The Assertive Community Treatment (ACT) Team works only with clients who have a severe and chronic Axis I diagnosis (schizophrenia and other psychotic disorders, major affective and bi-polar disorders). ACT clients typically are unable to engage in traditional mental health services, are non-compliant with medications, have numerous hospitalizations and demonstrate significant functional impairments in accessing safe, affordable housing, avoiding destructive lifestyle choices and have difficulty completing practical daily living tasks. The ACT team provides services both in a clinic setting and in the community. We assist clients in obtaining housing, financial assistance, accessing specialized medical services, navigating the legal system, etc. Further to this, we offer independent living skills training, recreational programming and one to one supportive counseling. |
